The FY 2025 ICD-10-CM code updates include 13 code descriptor revisions. Key examples include:

  • A7741: Changed from "Ehrlichiosis chafeensis [E. chafeensis]" to "Ehrlichiosis chaffeensis [E. chaffeensis]"
  • F9821: Changed from "Rumination disorder of infancy" to "Rumination disorder of infancy and childhood"

ICD-10-CM Coding Guidelines Updates

The 2025 ICD-10-CM Official Guidelines updates are minimal, primarily focusing on harmonization with new code introductions.

Status Code Updates

Category Z17 title expanded to: Estrogen, and other hormones and factors receptor status

  • Z17 - Estrogen, and other hormones and factors receptor status
  • Z18 - Retained foreign body fragments
  • Z19 - Hormone sensitivity malignancy status

New Guideline: Presymptomatic Type 1 Diabetes

Guideline I.C.4.a.1.(a): Codes E10.A- (Type 1 diabetes mellitus, presymptomatic) are assigned for early-stage type 1 diabetes that predates the onset of diabetic symptoms. Note: The age of a patient is not the sole determining factor, though most type 1 diabetics develop the condition before reaching puberty.

Significant Updates by Chapter

Chapter 2: Neoplasms

Codes in categories C81-C84 now have options to report "in remission." Categories C86 and C88 now have options for "not having achieved remission" or "in remission."

Chapter 4: Endocrine, Nutritional and Metabolic

Three new subcategories of codes have been added:

  • E10.A-: Type 1 diabetes mellitus, presymptomatic
  • E16.A-: Hypoglycemia level
  • E66.8: Obesity class

Several subcategories have new instructional notes directing reporting of additional codes for injectable non-insulin antidiabetic drugs (Z79.85) and/or hypoglycemia level codes.

Chapter 5: Mental and Behavioral Disorders

Eating disorder codes in category F50 (anorexia, bulimia, binge eating) have been expanded to specify severity. New codes specifically report PICA and rumination disorder in adults.

Chapter 11: Digestive System

Codes in category K60 (fissure and fistula of anal and rectal regions) have been expanded to specify complexity and nature of occurrence.

Chapter 13: Musculoskeletal System

Lumbar and lumbosacral intervertebral disc degeneration codes (M51.36 and M51.37) expanded to specify type of pain:

  • Discogenic back pain only
  • Lower extremity pain only
  • Discogenic back pain and lower extremity pain
  • Without mention of lumbar back pain or lower extremity pain

Chapter 19: Injury and Poisoning

Category T45 receives 6 new codes for poisoning by, adverse effect of, and underdosing of immune checkpoint inhibitors and immunostimulant drugs. Code T81.32 expanded with four new codes for greater specificity in reporting disruption of internal surgical wounds.

Chapter 21: Factors Influencing Health Status

Category Z17 updated with 8 new codes to indicate positive/negative receptor status and combined receptor status. New subcategory Z67.A- added for Duffy phenotype.
